Somebody wrote these under a name the roster does not know.
Every check of the form is this yours? compares the roster key on the record with the one on the session. When they stop matching, the records still render and the names still look right — what fails, quietly, is the author's ability to edit their own words, and any count of what they have already done.
This usually means an identity changed shape and the stored rows were not moved with it. Fix it with a migration, not by editing rows by hand.
